Annual Wellness Visit Services
A team of professionals with strong medical backgrounds collaborate to extract and identify all chronic medical conditions reported on each member.
- Chronic medical conditions identified via claims, medical chart review or based on suspect algorithms
- Medications, labs and any other source for potential suspect conditions are checked and presented on a checklist
- Checklist acts as a tool to help the PCP identify the history of medical conditions the member has and address them in the next face to face visit with the member.
Key components of an AWV:
As you know, quality health care begins with an annual wellness visit (AWV). AWVs help with health maintenance and prevention. To provide a complete checkup, you should include the following as part of your visit:
- Comprehensive physical examination
- Assess current medical conditions
- Complete appropriate lab work
- Address preventative care
- Review medications
- Promote a healthy life style
